What to Bring On Your First Visit

We know your time is valuable and that seeking treatment can be stressful.

So here’s what to bring to ensure you feel fully prepared when you arrive for your first visit.

  • For Adults 18 years or older: Please bring a picture ID, proof of household income, and any insurance information (if applicable).
  • For Children under the age of 18 or adults with a legal guardian: Please bring a parent or legal representative’s picture ID, child’s birth certificate, proof of household income, insurance information, and custodial/legal documentation (if applicable).
  • For Adults coming in for court-ordered evaluations: Please bring court-ordered paperwork, picture ID, proof of income, and insurance information if applicable
  • For all clients coming in for first-time appointments: Please bring any current prescriptions in the bottle or a list of current medications from the pharmacy, plus the following completed documents:

Insurance & Financial Information

Charlotte Behavioral Health Care is committed to making treatment as affordable as possible. Most Medicaid and Medicare HMOs are accepted, as well as private insurance, when applicable. For individuals without insurance coverage, a sliding fee scale is utilized for services, based on family size and income.

We accept most major insurance, including Florida Medicaid (Beacon Health Options, Staywell/Wellcare, Cenpatico/Sunshine State), Cigna, Aetna, and United Health Care. If you have questions about how your insurance applies, please review your insurance company’s website to see what your specific policy/plan offers, or you may discuss your options by contacting our Outpatient UM Department at 941-639-8300 ext. 2250 or the Recovery Center Assistant at 941-347-6444. Payment plans and sliding scales are also available.

As of 2/1/2023, CBHC will no longer be able to accept new Blue Cross/Blue Shield therapy and medication/psychiatric services. We are sorry we cannot assist you. If you are inclined to advocate on this issue, please contact your BC/BS network to request that additional CBHC licensed therapists be added to their network.
