Case Management
CBHC’s Case Management Services support individuals with accessing resources and services that build upon strengths with the goal of achieving stability and success in the community.
- Development of an individualized Service Plan that targets linkages to community resources and activities that support the Recovery of the individual and/or family.
- Face-to-face contact occurs a minimum every 30 days in the home and/or community. The actual amount of contact and support received is determined by the needs of each individual.
- Case Managers are available during normal business hours.
- Case Managers work with internal and external treatment and service resources to link consumers to these treatments/services as appropriate and monitor their effectiveness.
